USMLE Certification Updates

In light of current events and the IU School of Medicine Registrar office remote work status, we have implemented a temporary process change for certifying your USMLE Certification of ID forms.

We will be completing all forms electronically. The form you send us must meet the requirements below before we will be able to sign and submit it to the USMLE:

  1. Your form must contain your photo.
  2. Your form must contain your handwritten signature.

The USMLE has waived the requirement for you to appear in person to have your form certified, along with the requirement for the form to have the raised school seal over your photo.

Once you have completed your portion of the form and added your photo and handwritten signature, please scan and send it to the appropriate authorized signer for your campus (listed below).

One of the School’s authorized signers will sign your form electronically and send it to the USMLE on your behalf. You will receive an email from the authorized signer confirming that your form has been sent to the USMLE.

Regional campus students will need to send their forms to their campus authorized signers. You can find the authorized signer contact(s) for each campus listed below.
